1.   "Edit Online" per click kren. Change Form open ho jae ga.

2.  Sirf apna data change kren Baaki wese hi likha rehne den. Edit krne ka tareeka neeche Tasveer mein dia gya hy.

3.   (Ctrl+P) dba kr print le len. (Print Setting: Legal page lgaen aur Landscape mein print kren)

3. (Ctrl+P) dba kr print le len. (Print Setting: Legal page lgaen aur Landscape mein print kren)

1. Click "Download Excel", it will immediately download an Excel file

2.  Click ‘Edit file’ option in yellow bar at top, Edit the data according to your details

3.   Press (Ctrl+P) to print the page (Legal page in Landscape is highly recommended)

FORM Submission GUIDE:

1. After you get a print of your Bank Account Change Form, your own sign and stamp are required at the bottom-left side (= Prepared by), then you get this form signed and stamped by the Head of your institute (e.g., Principal of a college) on the bottom-right side (= Entered/Verified by) of your printed page. Keep the central line (= Audited/Checked by) blank for Accounts Office use.

2. Submit signed copy pinned with attachments (previous bank NOC, New bank account maintenance Certificate and a copy of latest pay slip) to your concerned Accounts Office.

Note: CNIC and bank details must be verified before submission.
